1. You receive an easy-to-use digital home blood pressure monitor (cellular-enabled).

2. Each time you check your blood pressure at home, the device records the data and transmits it to our office.

3. Our care team reviews your data for trends, abnormalities, or urgent warning signs and reaches out with adjustments to your management plan as needed.
